    Employee Health and Welfare Benefits

    Petaluma City Schools offers employees a comprehensive benefits package including a District-paid contributions for medical, dental, vision and life insurance.  Access the District's benefit eligibility chart, benefit summaries and rate information by clicking on the links below.  District-paid contributions are pro-rated based on FTE or hours per day.    

     

  • Rate, Eligibility and Verification Documentation

    Benefit eligibility is based on employment classifications and the FTE or work hours per day.  The current benefit eligibility chart may be accessed by clicking on the link below.

    Benefit Eligibility Chart

    The premiums for all Kaiser and Blue Shield plans (except for Anchor Bronze), Delta Dental and Vision Service Plan are based on composite rate pricing, which means the monthly premium is the same for every employee regardless of the number of family members covered by the plan.  Blue Shield Anchor Bronze is based on tiered rate pricing, which means the monthly premium is based on the specific number of family members covered by the plan (single, double or family).  The current rate chart may be accessed by clicking on the link below.

    The rate chart is based on the District-paid contribution for a full-time employee that is 100% benefit eligible.  Part-time employees receive a pro-rated amount of all District-paid contributions for medical, dental and vision.  To determine the over cap amount for a part-time employee, deducted the pro-rated cap amount from the cost per month as listed on the chart below.

    The District provides dental coverage through Delta Dental of California.  This coverage provides a maximum of $1,500 per enrollee for services each calendar year.  This group does not provide an ID card.  The group number for this plan is 6748-0230.  The ID number for this plan is the SSN for the subscriber.

    Vision Service Plan

    The District provides vision coverage through Vision Service Plan (VSP).  This coverage is available only to those employees that have enrolled on Kaiser Low, Blue Shield (any plan) or has opted-out of medical coverage (part-time employees only).  This coverage provides a variety of services per enrollee for services each calendar year.  This group does not provide an ID card.  The group number for this plan is 0010-0010 under contract number 00102093.  The ID number for this plan is the SSN for the subscriber.

    Vision Service Plan Benefit Summary

    Life Insurance

    The District provides a $37,000 life insurance policy to all eligible employees.  Teachers working in an elementary job share are provided a $18,500 life insurance policy.  Voluntary supplemental life insurance policies are available to all new hires and during open enrollment on an annual basis.
